We are seeking an experienced Care Coordinator to lead and coordinate safe, person-centred care for participants with complex needs across disability, aged care, and community health programs.
This role plays a pivotal leadership function, combining clinical oversight, care coordination, workforce supervision, and operational accountability. You will act as the primary point of contact for allocated participants while providing line management and professional support to Disability Support Workers.
This position reports to the Clinical Services Manager – Aged Care & General Health / Mental Health & Behaviour Support and is based in Karratha.
Develop, implement, and review individualised Care and Support Plans aligned with participant goals, clinical best practice, and NDIS requirements
Coordinate multidisciplinary care, including liaison with allied health, medical practitioners, behaviour support practitioners, plan managers, and families
Complete End of Plan Reports within required timeframes and monitor participant progress against agreed outcomes
Provide line management and supervision to Disability Support Workers, including supervision, performance reviews, competency assessments, and rostering
Maintain high standards of clinical documentation, compliance, and reporting using care management systems
Monitor service delivery quality through audits, incident management, and continuous improvement activities
Undertake limited direct care shifts to maintain participant rapport and clinical oversight
